If you have been involved in a motor vehicle crash in which a person is injured or damage to any one vehicle or property is in excess of $1000.00 you are required to complete a Massachusetts Motor Vehicle Crash Operator Report. Motor Vehicle Crash Operator Report copies are also available at any police station or Massachusetts RMV branch.
You will need to make three copies of your completed form, one for local police where the crash occurred, one copy for the Massachusetts RMV and one copy for your insurance company. Please note that if the crash occurred on a State roadway (including but not limited to: Route 93, Fellsway, Fellsway West, Middlesex Avenue, Elm Street, Roosevelt Circle, and Route 16) you should submit the completed form to the Massachusetts State Police. You have five business days to complete and file these forms.
If you would like a copy of the officer’s crash report you may request through the public records request form, in person, by postal mail, by fax, or through electronic mail.
